New Patients

Thank you for considering Community Physical Therapy for your rehabilitation needs. Below is a brief description of what you can expect during your initial visit to our office.

During your first visit to our office a NYS Licensed Physical Therapist will conduct a thorough evaluation of your condition and provide a brief treatment. Initial visits usually range from 1 to 1 ½ hours. Follow up visits are usually 45 minutes to 1 ½ hours. Treatment times may vary depending on your condition.

As a courtesy to our staff and your fellow patients we require that everyone arrive promptly for their visits and provide at least 24 hours notice for all appointment changes or cancellations. Failure to do this may result in increased waiting times or an inability to receive treatment.

It is recommended that you wear clothing that allows exposure of the injured area (i.e. baggy t-shirts or tank tops for shoulder injuries and shorts or baggy pants for knees). Skirts or dresses are not advised. Proper attire will allow your therapist to evaluate your condition more effectively. Changing areas are available for those who need them.

Paperwork

Please bring all pertinent reports regarding your condition to the first visit. This includes doctor’s prescriptions, MRI or X-Ray results, etc. You will also need to provide all insurance information such as your insurance card or claim number. Prior to your initial treatment, you will be required to fill out a few forms. You may complete them upon arrival for that visit or download them and complete them at your leisure (just don’t forget them at home).
